Educational tools

Search terms, planner selections and worksheet checkmarks are held in the current page’s browser memory. These tools do not submit those selections to an agent or save them to an account. Downloading a checklist creates a file on your device; printing uses your browser’s print controls.

This release does not add advertising pixels, session replay or behavioral lead scoring to the tools. Do not enter or share medical records, Medicare numbers, Social Security numbers, VA claim numbers or medication lists through ordinary public inquiry channels.

Ordinary website and hosting records

Delivering the website requires the browser and hosting infrastructure to exchange technical information. The hosting provider may process access and security records such as IP addresses, request times, requested pages, browser details and error information. Private use of a worksheet does not mean that the act of visiting a website is technically anonymous.

Links and buttons can navigate to another page. A downloaded or printed resource may be stored by your device, browser, printer or software under your control.

Website form-submission status

Website form submissions are not enabled through this preview. The site does not display a personal-information form unless the receiving business, contact information, service states, retention description and delivery checks are configured and verified.

The request endpoint rejects submissions while this gate is closed. We do not claim that an unverified email address or agent network is receiving requests.

Security and scope of a request

If activated, the form is limited to a specific requested contact. It does not accept attachments or free-text medical histories. It uses server-side validation and a time-limited form token, and it does not write the submitted personal fields to application console logs.

Technical safeguards reduce risk but do not make any system risk-free. A later insurance discussion, enrollment or document exchange needs its own appropriate process and disclosures.
